What Causes Acne and How Is It Assessed?
The key mechanisms involved in acne development and the main considerations during dermatologist assessment.
Acne is a common dermatological condition arising from the interaction between sebaceous glands, hair follicles, and the microbial balance on the skin surface. Although frequently seen during adolescence, it can persist into adulthood. This article explains the general mechanisms of acne development and the key points examined during dermatologist assessment — it does not provide a diagnosis or treatment recommendations.
What factors contribute to acne development?
Several mechanisms work together in acne development. Increased sebaceous gland activity, cellular accumulation at the follicular opening, bacterial proliferation, and the resulting inflammatory response are the core components of the process.
These factors play different roles in each individual. For this reason, even acne presentations that look similar may require different assessment and planning approaches.
Possible triggers and aggravating factors
- Hormonal fluctuations (puberty, menstrual cycle, certain hormonal conditions)
- Genetic predisposition and family history
- Comedogenic or potentially irritating skincare products
- Side effects of certain medications on the skin
- Stress, sleep patterns, and lifestyle factors
This list is for general information only. The cause of your acne can only be determined through physician assessment.
How are symptoms classified?
During dermatologist assessment, lesion type and distribution form the foundation of planning. Lesions can appear in different forms; severity varies from person to person.
Common lesion types
- Blackheads (open comedones) and whiteheads (closed comedones)
- Red, tender papules and pustules (inflammatory spots)
- Deeper nodules and cyst formations
- Post-acne redness or residual scarring
What is examined during dermatologist assessment?
Acne assessment is not limited to examining the skin surface. The physician considers lesion distribution, inflammation level, and any co-existing conditions as a whole.
- Lesion type, density, and affected areas
- Current and previously used skincare products
- Medication history and known allergies
- Menstrual cycle and hormonal history (in female patients)
- Previous treatment or procedure experience and responses
- Expectations and impact on daily life
Additional investigation or a period of observation may be recommended based on assessment findings. Treatment may not be planned on the same day in every case.
Are active acne and scar management addressed the same way?
No. During active inflammation, the priority is controlling existing lesions. Scar or discolouration management is generally planned once active acne is stabilised.
Early scar-focused interventions may not be appropriate while active lesions persist. This distinction is an important part of physician assessment.
How does personalised planning proceed?
A treatment or management plan is created by the physician, taking into account acne severity, skin tolerance, age, and personal expectations. The plan may be phased and updated throughout the process.
Response times vary from person to person; results within a specific timeframe cannot be guaranteed. Regular follow-up is important for safe and effective progress.
General principles in planning
- Assessment first, then planning — treatment suitability is determined following consultation
- Active and scarring phases are addressed separately
- Skin tolerance and potential side effects are monitored
- Patient information and consent are integral to the plan
